GENERAL SUMMARY AND SCOPE
Under the direct supervision of the Vice President, Operations, the Clinical Dietitian conducts nutritional assessments, plans, directs, and educates the diabetic patient population.

ESSENTIAL TASKS, DUTIES AND RESPONSIBILITIES
Reviews medical orders for all diets for patients with diabetes.
Participates in maintaining the comprehensive Diabetes Self-Management Education program which meets the requirements for Education Program Recognition by the American Diabetes Association.
Works collaboratively with the interdisciplinary education team. Collaborates with primary care physicians regarding therapeutic problem-solving of program referrals.
Coordinates Diabetes Self-Management Education curriculum and activities and provides direct diabetes education for individuals and groups. Coordinates and maintains all required documentation in program participant’s education record and ensures confidentiality.
Promotes healthy living and diabetes awareness and prevention by providing educational programs to the community such as community health fairs and diabetes support group.
Interviews patients and/or family to obtain information regarding food habits and preferences for guidance in planning the patient’s diet.
Screens medical records for pertinent information and documents as needed per established guidelines.
Calculates diets for diabetic patients and other modified diets, and provides diet counseling for all patients on modified diets who require it.
Charts appropriately in the patient's medical records after each patient contact.
Consults out patients upon physician's request.
Serves as resource person in nutrition care for medical and nursing care. Provides nutrition education for public programs, patients and staff, as required.
Regularly demonstrates the ability to serve patients of all ages in the routine performance of duties.
OTHER TASKS, DUTIES AND RESPONSIBILITIES
Keeps abreast of developments in clinical practice and serves as a patient advocate for current standards of diabetes care.
Demonstrates knowledge of current practice standards and guidelines and is able to communicate the knowledge to patient care team.
Participates in department performance improvement activities as assigned and attends required training.
Represents FRMC to the community. Acts as an ambassador and advocate for FRMC.
Assists with special functions, as needed.
Demonstrates service excellence and ACE.
Performs other duties, as assigned.
KNOWLEDGE, SKILLS AND ABILITIES
Registered Dietitian - licensed in State of Ohio. Certified Diabetes Educator (CDE) or goal of obtaining certification within 3 years of employment at Firelands.
Experience in diabetes care and education strongly desired.
Ability to read and write small letters and numbers, and ability to perform basic arithmetic.
Ability to communicate and understand both written and oral communication.
Ability to work in a safe and efficient manner and maintain an accident free work place, including ability to demonstrate a working knowledge of Medical Center emergency codes.
Ability to comply with OSHA regulations and CDC standard and transmission-based precaution recommendations and to utilize proper personal protective equipment. Ability to comply with provisions of applicable S.D.S. forms.
Successful completion of a one hundred eighty (180) day get acquainted period.
Demonstrated ability to work independently and as an effective member of a multi-disciplinary team.
Must possess a valid State of Ohio motor vehicle operator's license and insurable under Firelands auto policy. Employee must provide proof of personal automobile liability insurance coverage upon request.
