The Social Worker will conduct and prepare psychosocial assessments, offer therapeutic interventions and contribute to the discharge planning process for patients and families receiving services through residential programs at the Lindner Center of HOPE. In addition, social worker will assist patients and/or their families/responsible parties in coping with issues related to pre-admission and admission.
Major Duties and Responsibilities: 1. Provide Social Work services for Residential Unit Intake process.
Conduct and prepare psychosocial assessments for patients inquiring about services on the residential units at the LCOH. Through the assessment process, emotional, social, and environmental strengths and challenges will be identified and acknowledged, while addressing patient/family/responsible party needs related to diagnosis, illness, treatment, and/or life situation. Determines if needs of the patient can be met at LCOH. Makes recommendation to the treatment team about whether a patient should be admitted.
Reviews pricing information and payment expectations with patients and families prior to admission. Discusses various financing and payment alternatives to help them determine how they can pay for these private pay services.
Participate in the LCOH clinical team which contributes to the formulation, development, and implementation of the patient’s treatment plan utilizing appropriate social work interventions. Interventions may include, but are not limited to, crisis intervention, brief therapy, cognitive behavioral and dialectical behavioral principles relating to individual, family, and group therapy.
Documentation in the Electronic Medical Record and paper chart. Documentation is timely, legible, concise, and pertinent and meets the profession’s and department’s documentation standards. Oral reporting is clear, concise, relevant, and timely. Perform all required data collection for the department accurately and within expected time frame.
Address discharge planning needs across a multidisciplinary care continuum which includes interacting with consulting physicians and other health care providers. Distributes discharge summary packets post discharge to patients. Serves as a resource to patients and families in accessing post-discharge services based on resource contacts and relationships. Maintains excellent relationships with external referral sources.
Provide education to patients and families around issues related to the adaptation of the patient’s illness and/or life situation during admission process. A ddresses concerns about treatment, i dentifies the benefits of the treatment and various programs, relevant to the patient’s individual situation .
Participates in clinical supervision on a scheduled basis.
Maintain working knowledge of and collaborates with community agencies; participate in area activities related to health and social services while providing brief overviews to colleagues on clinical education attended .
Works closely with Marketing and Outreach staff, participating in marketing and outreach activities as requested. Works seamlessly with external relations marketing and outreach to facilitate admissions and cultivate referrer relationships. May include occasional travel.
Maintain licensure and pursue current information related to mental health and social work. Participate in continuing education opportunities to further enhance job related knowledge.
Participates in department and LCOH quality improvement efforts and peer review standards are met.
Meets competency skills for evaluations in area of practice.
Knowledge and Skills:
Master of Social Work degree from an accredited school of social work preferred. Master Degree in related field (i.e. social work, counseling, psychology) required
Licensed Social Worker or Licensed Independent Social Worker by the State of Ohio Social Work Board required.
At least 3 years’ experience in a health care setting; Psychiatric/mental health experience required.
Solid knowledge of social work practice; strong assessment, interview and counseling skills.
Demonstrates the knowledge and skills necessary to provide age-specific and developmentally appropriate care for the spectrum of populations admitted to LCOH.
Strong interpersonal skills; ability to collaborate well.
Ability to enthusiastically follow and model the Lindner Center of HOPE mission, vision, and values.
